Facial Plastic Surgery in San Francisco

Given enough time, we are all faced with wrinkles, lines, and other signs of facial aging, from sagging along the jawline to heavy, tired-looking eyelids. Facial plastic surgery can restore youthful contours and balance to the face, neck, eyes, and nose.

Double board-certified plastic surgeon Dr. Leonard Gray offers surgical facial procedures designed to revive confidence and self-image, with a focus on natural results rather than an overdone or pulled appearance. For patients who prefer less downtime, he also offers non-surgical facial rejuvenation.

What Types of Facial Plastic Surgery Does Dr. Gray Offer?

Dr. Gray offers three facial plastic surgery procedures, each designed to address a different area of the face. Select a procedure below to learn more.

Facelift

A facelift, often combined with a neck lift, lifts sagging skin and deeper tissues of the lower face and neck to smooth jowls and restore a defined jawline. Dr. Gray tightens the superficial musculoaponeurotic system (SMAS) beneath the skin for longer-lasting results without a pulled or windblown look, and he places incisions where they are well hidden, even when hair is worn up.

Eyelid Surgery

When droopy lids or under-eye bags have you looking tired and worn, blepharoplasty is designed to refresh the eyes. Eyelid surgery removes or repositions excess skin and fat from the upper and lower eyelids, and Dr. Gray uses magnification and precise measurements to place incisions within natural creases, where they are difficult to see. Most patients return to work within 7 to 14 days

Rhinoplasty

Better known as a nose job, rhinoplasty improves nose structure, balances size, or opens the nasal passages for better breathing. It commonly corrects size, length, a bulbous or drooping tip, or a dorsal hump. Dr. Gray uses an open or closed approach depending on your anatomy and goals, and the nasal splint is typically removed within 7 to 10 days.

Facial Plastic Surgery FAQs

The right procedure depends on the area that concerns you most. A facelift addresses sagging skin, jowls, and loose skin along the jawline and neck, eyelid surgery treats heavy upper lids and under-eye bags, and rhinoplasty reshapes the nose or improves breathing. Dr. Gray will evaluate your features and goals during your consultation and recommend the best approach.

Yes. Dr. Gray’s facelift plans address both the lower face and neck, tightening lax skin and reducing jowls. He can also improve vertical neck bands by tightening the platysma muscle with corset stitching.

Natural results are Dr. Gray’s priority. His goal is a refreshed version of your own appearance, without an over-pulled face, a change in the natural shape of your eyes, or a nose that looks out of balance with your features.

Most patients enjoy their facelift results for a decade or longer. You will continue to age naturally, and longevity depends on skin quality, lifestyle, and genetics.

Yes. Eyelid surgery is often combined with a facelift for more comprehensive rejuvenation, and many patients pair surgery with non-surgical treatments such as Botox® or dermal fillers.

Yes. Dr. Gray offers non-surgical facial rejuvenation options, including Botox and Xeomin, dermal fillers, FaceTite, laser resurfacing, and thread lifting. Fillers can also be used for a non-surgical rhinoplasty in select patients, though surgery offers more significant and permanent results.

It can. For patients with excess upper eyelid skin that hangs over the lash line, removing that skin can open up the field of vision in addition to creating a more rested appearance.

No. Rhinoplasty can be performed for cosmetic reasons, to improve breathing through the nose, or both. Dr. Gray performs the procedure for each of these goals.

Dr. Gray performs facelift, eyelid surgery, and rhinoplasty under general anesthesia in Bay Area Plastic Surgery’s AAAASF-accredited surgical suite. These procedures are typically outpatient, though some facelift patients stay overnight for monitoring.

There is no single right age. Many facelift patients are between 50 and 70, while some patients in their 40s are also good candidates, depending on their skin and signs of aging.

Cost depends on the procedure, the complexity of your surgery, and whether procedures are combined. Dr. Gray and his team will provide a detailed estimate after your consultation, and financing options are available.
