What Can I Expect in Recovery?

Dr. Gray wants all of his patients to understand that while the results are well worth it, it is a relatively challenging recovery to undergo. That said, the Bay Area Plastic Surgery team will be there to help you along every step of the journey. Most patients follow this general timeline for recovery:

Immediately After

After the surgery has concluded, you will be taken to a recovery room and monitored until you awaken on your own. Because a tummy tuck is an outpatient procedure, you can return home the same day with the help of a friend or family member. Dr. Gray typically uses a powerful numbing solution, Exparel®, during surgery to reduce post-procedure pain. Working for up to four days after the operation, Exparel makes patients significantly less sore than past methods.

First Three Weeks

Dr. Gray instructs most of his patients to remain in a bent-over position for the first three weeks post-op. You will receive a walker, pain medications, and detailed instructions on what to do and not do during this time frame.

Two Months

You will need to avoid strenuous exercise and heavy lifting for two months. This allows your incisions to strengthen and heal correctly and helps prevent complications. As Dr. Gray tells his patients in consultation, the lifting limit for this period will be about as much as a cup of coffee. You can still get up and move around when Dr. Gray clears you, but this will primarily be limited to flat walking.

Four Months

You should see most of the swelling dissipate in about four months.

How is a Tummy Tuck Performed?

Each tummy tuck will be customized to the individual. That said, Dr. Gray follows some necessary steps for each operation:

You will be asleep under general anesthesia for the entire procedure, which is performed in Bay Area Plastic Surgery’s AAAASF-accredited surgical suite.

Initially, Dr. Gray will use liposuction to remove excess fat around the waist, hips, and tummy. This will help provide the most contoured end result. After liposuction, he will begin the tummy tuck operation.

Dr. Gray will create a small incision in the pubic area. He will make these incisions in the best spot to ensure that any future scarring will be minimal and concealable.

After making the incisions, Dr. Gray will separate the abdominal skin from the muscle all the way up to the chest.

Next, Dr. Gray will tighten stretched abdominal muscles by suturing them together. These specialized sutures will bring bulging muscles back into a more natural, straight up-and-down position.

The next step in the process is to excise loose skin. Dr. Gray will use his specialized surgical table, which allows the patient to safely move to a sitting position during the surgery, to allow him to review and approve each marking before removing any skin. Once checked, all of the extra skin is extracted. Dr. Gray will insert a drain at this time.

While you are still in a sitting position, Dr. Gray will carefully connect and suture the remaining skin flaps. Dr. Gray’s methods will not leave any excess skin or “dog ears.”

Once the skin is pulled down and connected in the correct areas, Dr. Gray will recreate your belly button. He will make a tiny incision, remove a wedge of skin, and then carefully sew the belly button into this spot. Dr. Gray uses this method to get a belly button that tucks in on the bottom and hoods over on top—simulating an aesthetically natural look.

An extended tummy tuck removes excess skin, fat, and tissue from the abdomen and the flanks. It is similar to a traditional tummy tuck, but the incision extends further around the waist toward the hips to address loose skin on the sides. This allows for a more comprehensive change to the overall appearance of the midsection. Dr. Gray often recommends an extended tummy tuck for patients who have experienced significant weight loss, pregnancy, or aging and have excess skin that wraps around the sides of the abdomen.

Often, yes, if enough loose skin remains. The revisions Dr. Gray sees are usually after surgery has been performed elsewhere. Common problems are leftover skin and stretch marks; a scar that sits too high or looks irregular; muscle separation that was not fully repaired; or a belly button that looks too high, too scarred, or off-center.

When there is still skin to work with, revision means doing the tummy tuck over: a lower, shorter scar, a complete muscle repair, and a more natural umbilicus. If too much skin was already removed, a full redo may not be possible. That is determined on exam.

A tummy tuck revision is a complete redo, not a small scar touch-up. Dr. Gray confirms there is enough skin, then resets the scar lower and shorter, repairs the separated abdominal muscles, removes leftover lower-abdominal tissue, and rebuilds a more natural belly button.

  1. The abdomen is marked so that the new scar sits as low and short as swimwear will cover.
  2. The skin flap is lifted to the breastbone and out to the sides, and the belly button is freed from the flap.
  3. With the muscles relaxed, the rectus separation is repaired from the breastbone to the pubic bone.
  4. The patient is then raised to a seated position to check the markings. Leftover skin is removed, and the tissues are closed in layers.
  5. A single drain is placed low in the pubic area. A long-acting numbing medication (Exparel) is used in the repair and incision lines.
  6. A new opening is made for the umbilicus, the belly button is reshaped and sewn in place, and the incisions are sealed with surgical glue that lasts about four weeks.


The goal is a lower scar, a tighter abdominal wall, and an umbilicus that looks as close to natural as the tissues allow.

Often, yes. If your abdomen has changed since your first tummy tuck, for example after another pregnancy or weight gain, Dr. Gray can evaluate whether a second operation can re-tighten the muscles and remove excess skin and fat. Nothing about this surgery will harm you if you decide to have children down the road. However, it is recommended that you try to have this surgery when you are finished having children, as the results may be lost with a new pregnancy or weight gain.

A full tummy tuck treats loose skin, stretch marks, and muscle separation from the breastbone down to the pubic bone. A mini tummy tuck treats a smaller problem: muscle separation only from the belly button to the pubic bone, in a patient who does not have much loose skin or many stretch marks.

The mini incision is short, about 10 centimeters. The repair stops at the umbilicus. It does not remove much skin, and it does not fix the higher muscle separation above the belly button.

A full tummy tuck candidate is not a mini candidate. When the anatomy matches (lower diastasis only and little extra skin), a mini tummy tuck can be an excellent operation. The difference is matching the procedure to the problem, not choosing a smaller surgery for a bigger one.

A tummy tuck can tighten separated abdominal muscles and remove loose skin in men who have those findings. The indications are the same as for women: extra skin, stretch marks, and muscle separation from the breastbone to the pubic bone. That anatomy is most common after major weight loss.

If a man does not have loose skin or muscle separation, he is usually a better candidate for liposuction than for a tummy tuck. Liposuction can be combined with a male tummy tuck, but the upper abdomen is treated more conservatively because that tissue is the flap and its blood supply has to be protected. The waist, flanks, and hips can still be contoured more fully.

Liposuction cannot remove hanging skin or repair diastasis. A tummy tuck can.

Yes. Repairing rectus diastasis (the separation of the abdominal muscles) is one of the main parts of a tummy tuck. After the skin is lifted, the muscles are brought back together from the breastbone to the pubic bone, so the midline gap closes and the abdominal wall lies flatter.

If an umbilical hernia is present, it can usually be repaired in the same step. When the separation exists only from the belly button down, a mini tummy tuck can repair that lower segment. Liposuction does not repair muscle separation.
